What is personal health information?
Personal health information includes any identifying information about you that relates to your physical or mental health, the care you have received, or your past or current health-care providers.
How we collect and use it
Charis Counselling & Clinical Services (Iddi Zackari, RP) collects personal health information directly from you for the purpose of providing psychotherapy services. We use this information to assess your needs, plan treatment, document your care, and bill any extended health insurance you may submit claims to.
How we protect it
Records are kept in a secure, encrypted system that meets PHIPA requirements. Access is restricted to Iddi Zackari, the practitioner. Records are retained for the period required by professional standards (currently a minimum of 10 years from the date of the last interaction, or 10 years from the age of majority for minors).
Limits of confidentiality
In limited circumstances, Ontario law requires us to disclose personal health information without your consent. These include:
- When there is reason to believe a child is being abused or neglected
- When there is imminent risk of serious harm to you or another person
- When required by a court order or subpoena
- When a regulated health professional sexual abuse disclosure is required by the Regulated Health Professions Act
Your rights
Under PHIPA, you have the right to:
- Access your records
- Request corrections to your records
- Withdraw consent for the use or disclosure of your information (subject to legal and professional limits)
- File a complaint with the Information and Privacy Commissioner of Ontario
